DISTANCE SALES AGREEMENT

This Distance Sales Agreement (“Agreement”) is made and entered into as of [Date] by and between [Seller Name], located at [Seller Address] (hereinafter referred to as “Seller”), and [Buyer Name], located at [Buyer Address] (hereinafter referred to as “Buyer”).

1. Subject Matter

2. Purchase Price
